15例后骨间神经病变患者的神经电生理特点Neuroelectrophysiological characteristics of 15 patients with posterior interosseous neuropathy

摘要(Abstract):
目的探讨后骨间神经病变患者的神经电生理特点。方法纳入15例后骨间神经病变患者,对所有患者进行针极肌电图和神经传导检查。观察和记录指总伸肌、尺侧腕伸肌、肱桡肌、肱二头肌、三角肌、拇对掌肌、小指展肌是否有纤颤电位、正锐波电位,运动单位电位(motor unit action potentials,MUAPs)的时限和波幅.以及募集相MUAPs的发放密度;检测桡浅神经、正中神经、尺神经感觉神经电位(sensory nerve action potential,SNAP)波幅和感觉神经传导速度(sensory nerves conduction velocity,SCV);检测桡神经、肌皮神经、腋神经动作电位复合肌肉动作电位(compound muscle action potential,CMAP)波幅和末端潜伏期(distal motor latency,DML);检测正中神经、尺神经动作电位的DML、运动神经传导速度(Motor nerve conduction velocity,MCV)及CMAP波幅。应用独立样本t检验研究患侧与健侧桡浅神经SCV及SNAP波幅、患侧与健侧桡神经动作电位DML及CMAP波幅的差异。结果患侧与健侧桡神经动作电位DML(指总伸肌记录,t=0.217,P=0.831;尺侧腕伸肌记录,t=1.852,P=0.085;肱桡肌记录,t=-1.726,P=0.106)差异无统计学意义;患侧与健侧桡神经CMAP波幅(指总伸肌记录,t=-44.613,P=0.000;尺侧腕伸肌记录,t=-38.751,P=0.000)差异有统计学意义;患侧与健侧桡神经CMAP波幅(肱桡肌记录,t=-0.056,P=0.956)差异无统计学意义;患侧与健侧桡浅神经SCV及SNAP波幅(t=-1.035,P=0.318;t=-1.196,P=0.252)差异无统计学意义。结论后骨间神经病变患者患侧桡神经动作电位DML(指总伸肌、尺侧腕伸肌记录)正常,CMAP波幅降低,桡浅神经SCV及SNAP波幅正常。
