伴枕区痫样放电癫痫患儿认知功能损害状况及影响因素分析Cognitive impairment status and influencing factors of children with epilepsy and epileptiform discharge in children

摘要(Abstract):
目的观察伴枕区痫样放电癫痫患儿的认知功能损害情况,探讨认知功能损害的影响因素。方法回顾性分析濮阳市油田总医院2018-07—2019-07治疗的258例癫痫伴枕区痫样放电患儿的临床资料,评估患儿认知功能损害状况,将存在认知功能损害患儿纳入认知功能损害组,未发生认知功能损害患儿纳入无认知功能损害组,收集患儿临床资料,采用单因素及多因素分析患儿认知功能损害的影响因素。结果 258例癫痫伴枕区痫样放电患儿中19例发生认知功能损害,认知功能损害发生率7.36%;单因素分析显示发病原因、病灶部位、社会家庭关爱与癫痫伴枕区痫样放电患儿认知功能损害无相关性(P>0.05);起病年龄、病程、发作类型、用药数量、发作频率及发作持续时间可能是癫痫伴枕区痫样放电患儿认知功能损害的影响因素(P<0.05);将单因素分析得到的可能影响因素采取Logistic回归分析,结果显示起病年龄≥6岁、病程1~5 a、全面性发作、多种药物治疗、发作频率≥4次/月、发作持续时间是癫痫伴枕区痫样放电患儿认知功能损害的危险因素(OR>1,P<0.05)。结论癫痫伴枕区痫样放电患儿认知功能损害受起病年龄、病程、发作类型、用药数量、发作频率及发作持续时间影响,可针对上述影响因素,尽早为癫痫伴枕区痫样放电患儿采取积极合理的干预,以预防认知功能损害发生,对改善患儿预后可能有积极意义。
