血清CXCL9及血小板参数与老年脑梗死患者神经功能缺损的相关性Correlation of serum CXCL9 and platelet parameters with neurological deficits in elderly patients with cerebral infarction

摘要(Abstract):
目的观察老年脑梗死患者血清趋化因子C-X-C基序配体9(CXCL9)及血小板参数表达状况,并分析各参数表达与脑梗死患者神经功能缺损的关系。方法选取2018-01―2019-12濮阳市中医医院收治的89例老年脑梗死患者为研究对象,根据美国国立卫生研究院卒中量表(NIHSS)评估神经功能并分组;另选取同期本院体检中心证实健康的80例健康体检人群作为对照组;检测并比较受检者血清CXCL9、血小板参数[血小板计数(PLT)、平均血小板体积(MPV)、血小板分布宽度(PDW)]水平及各参数,分析其与老年脑梗死患者神经功能缺损(NIHSS评分)的相关性。结果脑梗死组血清CXCL9、MPV、PDW水平高于对照组,PLT水平低于对照组(P<0.05);89例老年脑梗死患者中,神经功能缺损轻度、中度、重度分别为33、32例、24例;中度组、重度组血清CXCL9、MPV、PDW水平均高于轻度组,PLT水平低于轻度组;且重度组血清CXCL9、MPV、PDW水平高于中度组,PLT水平低于中度组(P<0.05);相关性检验结果显示,脑梗死患者神经功能缺损(NIHSS评分)与血清CXCL9、MPV、PDW呈正相关(r>0,P<0.05),与PLT水平呈负相关(r<0,P<0.05);血清CXCL9及PLT、MPV、PDW用于预测老年脑梗死患者重度神经功能缺损的曲线下面积分别为:0.804、0.951、0.833、0.841,均有一定预测价值,且以PLT的预测效能较突出。结论老年脑梗死患者存在不同程度神经功能缺损,可能与血清CXCL9及血小板参数异常变化有关,临床可考虑通过监测老年脑梗死患者的CXCL9、MPV、PDW、PL水平T变化情况,来预测患者神经功能缺损程度。
