静止性脑梗死患者认知损害的影响因素研究Clinical related factors of cognitive impairment in patients with static cerebral infarction

摘要(Abstract):
目的探讨静止性脑梗死(SCI)的认知损害程度及其影响因素。方法应用韦氏智力量表,对经CT或MRI证实的143例SCI患者进行神经心理学测评,并与临床性脑梗死(CCI)组176例及正常对照组129例进行比较。对SCI组内智能障碍者与非智能障碍者的梗死病灶情况(数量、侧别、部位)、人口学情况(性别、年龄、文化程度、婚姻状况、慢性病史)进行统计分析。结果 SCI组23例存在智能障碍,其构成比与CCI组45例比较差异无统计学意义;智能障碍者在病灶数量(多灶)、部位(颞叶/侧脑室旁/丘脑)、文化程度(小学以下)、既往基础病史(糖尿病/甲状腺功能减退),与非智能障碍者有显著性差异(P<0.01)。结论静止性脑梗死,其认知障碍的程度与症状性脑梗死等同。多发性脑梗死、颞叶梗死、侧脑室旁梗死、丘脑梗死、小学以下文化程度、糖尿病史、甲状腺功能减退史是SCI认知障碍的危险因素。
