中性粒细胞与淋巴细胞比值 纤维蛋白原与急性脑梗死侧支循环建立的关系研究Relationship between NLR,Fib and establishment of collateral circulation in acute cerebral infarction

摘要(Abstract):
目的探讨中性粒细胞与淋巴细胞比值(neutrophil to lymphocyte ratio,NLR)、纤维蛋白原(fibrinogen,Fib)与急性大脑中动脉供血区梗死侧支循环建立的关系。方法纳入急性大脑中动脉供血区脑梗死患者109例,入院后完善磁共振DWI检查,应用DWI-ASPECTS评分将患者分为侧支良好组及侧支不良组,同时收集血常规、凝血功能等临床资料,进一步将NLR、Fib等指标与侧支循环进行相关性分析,利用ROC曲线探索其对侧支循环建立不良的预测价值。结果 2组糖尿病、梗死灶最大层面直径、预后不良、NIHSS评分、中性粒细胞计数、淋巴细胞计数、NLR、Fib比较差异有统计学意义(P<0.05)。中性粒细胞计数、Fib、NLR与侧支循环建立不良呈正相关,而淋巴细胞计数呈负相关。ROC曲线显示NLR、Fib及两者联合诊断中重度脑梗死的曲线下面积分别为0.750、0.650、0.782。结论 NLR、Fib与脑侧支循环建立有关,且高NLR、高Fib预示着侧支循环建立不良。
