Todd瘫痪全脑CT灌注成像1例及文献复习

摘要(Abstract):
目的加强对Todd瘫痪临床及影像特点的认识,避免临床误诊误治。方法回顾性分析1例Todd瘫痪患者的临床资料、脑部影像,并复习相关文献。结果本例患者因突发右侧肢体抽搐6h入院,间断抽搐伴偏瘫及失语,头颅CT排除出血,全脑CT灌注提示左侧大脑半球呈高灌注,排除大血管事件,完善MRI左侧侧脑室下脚新发点状梗死。予以抗癫痫药物治疗,癫痫症状控制后患者偏瘫恢复,头颅MRI不能解释患者体征及病情恢复,最终诊断为Todd瘫痪。结论 Todd瘫痪易误诊为急性脑梗死而导致溶栓错误,为有效鉴别,应及对完善相关影像学检查,除头颅MRI外,全脑CT灌注检查可应用于快速、准确识别Todd瘫痪,避免溶栓错误。
