程序化手法治疗脑卒中后肩痛患者临床观察Application of programmed manipulation in patients with shoulder pain post-stroke

摘要(Abstract):
目的观察程序化手法治疗脑卒中后肩痛(PSSP)对患者疼痛及运动功能的影响。方法纳入郑州大学第一附属医院康复医学科PSSP患者185例,观察组92例和对照组93例,2组患者均接受二级预防药物并合理管控血压、血糖、血脂等危险因素。采用常规神经促通技术促进瘫痪肢体的功能恢复,对日常生活注意问题,包括良姿体位摆放予以详细的指导和解答。观察组由运动治疗师予以指导、训练及宣教,具体有降低肌肉张力、关节活动度训练、软组织松动术、关节松动术。治疗前和治疗4周后分别对2组患者进行疼痛视觉类比法(VAS)和上肢运动功能评定量表(FMA-U)评定。结果观察组与对照组治疗前VAS评分、FMA-U评分比较差异无统计学意义(P>0.05)。治疗4周后2组患者VAS评分别为(1.21±1.12)分和(4.62±1.36)分,2组患者FMA-U评分分别为(32.45±5.25)分和(11.59±4.38)分。与治疗前相比,观察组和对照组治疗4周后VAS评分均显著降低,FMA-U评分均显著提高,差异有统计学意义(P<0.05)。观察组治疗后VAS评分下降和FMA-U评分提高程度均高于对照组,差异有统计学意义(P<0.05)。结论程序化的手法治疗能有效改善PSSP患者运动时的疼痛程度及偏瘫上肢运动功能。
