早产儿脑白质损伤颅脑超声检查时间的研究Study on the optimal time of brain uitrasonographic examination in preterm infants with periventricular white matter injury

摘要(Abstract):
目的探讨早产儿脑白质损伤最佳颅脑超声检查时间,以期及时发现病变,对临床治疗、干预提供依据,减少早产儿神经系统后遗症。方法选取揭阳市人民医院新生儿中心2014-06—2016-06收治的80例早产儿为研究对象,在不同的时间进行颅脑检查,观察不同超声时间点并分析颅脑超声检查的最优时间。结果 80例早产儿超声诊断:脑室周围白质回声全程正常42例及脑室周围白质回声增强38例。其中,脑室周围白质回声增强出现在624(13.8±1.5)h,中位数12.5h;回声最强出现时间为224(13.8±1.5)h,中位数12.5h;回声最强出现时间为25(3±1.4)d,中位数3d;回声增强持续时间为55(3±1.4)d,中位数3d;回声增强持续时间为514(9±2.5)d,中位数9d;脑室周围白质囊性变出现时间为1614(9±2.5)d,中位数9d;脑室周围白质囊性变出现时间为1622(18±3.8)d,中位数18d;在28d内未发现囊腔消失。结论颅脑超声检查早产儿脑白质损伤在不同时间点有不同表现,应在合适时间内及时进行影像学检查。
