短暂性脑缺血发作与后继脑梗死关系的初步探讨The study on the relationship between transient ischemic attacks and a subsequent cerebral infarction

摘要(Abstract):
目的探讨短暂性脑缺血发作与后继脑梗死的关系。方法选择首次入院脑梗死患者658例,其中同侧TIA的患者122例,按照脑梗死前是否预先发生同侧的TIA分为无TIA组和有TIA组。采用CSS和ADL评分标准,于入院时进行神经功能缺损程度评分,并于病程1个月时对各组进行日常生活能力评分,比较各组间神经功能缺损程度、预后。结果脑梗死前无TIA史患者中轻度神经功能缺损为332例(62%),有TIA组为94例(77%),中、重度神经功能缺损在无TIA组中为204例(38%),有TIA组为28例(23%),2组比较差异有统计学意义(χ2=9.94,P=0.002)。Ⅰ级预后脑梗死预先有TIA史88例(72%),无TIA史340例(63%),但二者相比差异无统计学意义(P=0.069)。结论脑梗死前发生的TIA可能对人脑产生缺血耐受作用,从而减轻随后严重脑梗死的神经功能缺损程度,改善脑梗死的预后。
